Form Section Type The Form Section Type creates online forms that can be used for registration, evaluation, information gathering, tests, and other applications. Respondents submit the form online. The data from each submission is collected as items. This data can be downloaded and used in several applications. Here is an example of a form as seen by the respondent: STEP 1 Click on the section under which the new section will appear. Click on Add a Child Section. Enter a Title for the new section. Select Form. Click on STEP 2 Click on for the new section title. ![]() STEP 3 Fill in Title and Introduction Click on ![]() STEP 4 Click on the red ball. It will change to green, turning on the section so it can be viewed on the site. ![]() STEP 5 Click on ![]() STEP 6 Click on A Step is a page. If there are portions of the form needing to be separated, there will be multiple steps. If you want all questions on one page (respondent scrolls down), there will be one step. ![]() STEP 7 Fill in Title and Alignment. Click on ![]() STEP 8 Click on ![]() STEP 9 Select a Field Type ![]() Examples: Select List creates a pull down list. Radio List allows only one selection. Check List Creates a list with boxes to check all that apply (allows more than one selection). Date and Time creates pull down menus to change the time and date. When finished click on ![]() STEP 11 The Confirmation Step allows the respondents to verify their answers before submitting their forms. To turn off confirmation, click on Hide Confirmation Step on the toolbar. ![]() STEP 12 The Submission Page is the last page a respondent will see after filling out a form. This could include a thank you or additional instructions. Click on Submission Page ![]() Then on Click Here STEP 13 Fill in the fields for the Submission Step Click on ![]() SPAMJAM USERS ALERT! If you included your email address in the Submitted Form Email field (see above) the submitted forms may go to your SpamJam. To prevent this from happening with future submitted forms: In your Lotus Notes inbox click on the SpamJam message. Once in the log click on the yellow icon by the message for the submitted form. Click Restore. Select Accept mail from this Domain. Click OK. If you want to view the submitted form on the same day it was sent- In your inbox click on the Actions menu>SpamJam>SpamJam log. Double click on the date of the submitted form. Proceed to Restore. (You could also go to administration for your site and select the form. Submitted forms will appear as items.) If the submitted form does not go into your SpamJam but turns up in your inbox with a question mark, select the submitted form, click on the Actions menu>SpamJam>Accept selected Domains. To see Videos on SpamJam go to: http://connect.spps.org/SpamJam_Help_Videos.html STEP 14 When finished with the form builder click on ![]() STEP 15 ONSITE After a respondent fills out a form online he/she will click on Finish ![]() STEP 16 ONSITE If a Confirmation Page was enabled in the Form Builder the respondent will check the answers and click on Back to make changes or on Confirm ![]() STEP 17 ONSITE The respondent will see the text that was entered under Submission Page in the Form Builder ![]() STEP 18 The submitted forms will appear as items in administration. These items can be viewed or deleted. (See below for downloading data.) ![]() STEP 19 To download the surveys in a comma delimited format click on Download Form Data Enter the start and end dates for the surveys you want to download. Click on ![]() STEP 20 Data in a comma delimited format can be viewed in a number of applications. If Excel is on the computer the downloaded data can open directly in Excel. ![]() |
